I can't recommend the best drug themed movie, as we all have our favourite vices, and like different things!
I for one, have many of them, so can't just choose one! ;)
But I'd like to recommend...
1. Trainspotting - For H, obvious and amazing! Although I don't do H, I was mindblown by this epic film! :)
2. Blow - For Coke, the obvious again and amazing - If you haven't seen it yet, where have you been? ;)
3. Requiem for a Dream - Four individuals, all fucked up on heroin, cocaine, and speed, and the problems that come with it, brilliant script, beautifully acted, simply superb! :D
Have any of you heard/watched a film called Dead Babies?
It's based on Martin Amis second novel, and is about a clash of British and American youth cultures, it's very engaging!
It has a mean streak and a devilishly dark side, if you like your dark humour you'll thoroughly enjoy it, and is refreshingly original!
One to watch! :)

To be honest I thought that Fear and Loathing (the film) followed the book extremely closely.
Apart from the bit where, in the book, he ends up in jail for the night, and a more detailed explanation of the shooters at the start of the bike race, I couldn't think of much that the film left out.
Usually there are entire chapters missing but in the case of Fear and Loathing, I thought Terry Gillingham did a fantastic job of turning the book into a film.
I read the book after seeing the film and TBH it was like reading the script of the film!

I like to watch all drug themed movies and documentaries no matter what sort of slant they are putting on it.
Here's the summary:

Another hilarious drugs related(ish) film, about a group of holiday reps doing their first season in Ibiza, their female manager, is a crazy coke head who does anything to get her fix!
If you liked human traffic you should definitely watch this! :)
SW9 came from the same producers as Human Traffic.
Whereas Human Traffic was set in Wales, SW9 is set in London, a much deeper film, much more street cultured!

Yeah I agree, it was very closely followed but there are certain parts which are so much more vivid and effective through the mastery of hunter s man. I don't mean to take away from the film as its my fav film of all time but you can pick up more of the satire through the book. Like the bit where raoul's at the petrol station asking where to find the American dream and it's a place where all the junkies and lowlife's hangout. That bit really sticks in my mind.
Although not a drug film/book the rum diary was good too.. Unfortunately they didn't follow the book quite as closely for that one.

Fear and Loathing is a great watch. Something about seeing two guys push their boundaries while having a wild crazy time doing it is just so fun to watch.
Also, Blow. Any movie that can make you want cocaine that bad after watching is good in my books.

There's a pretty good DMT documentary called The Spirit Molecule which is definitely worth a watch and'll likely put get you scouring the SR DMT vendors. Focuses on both freebasing DMT and ayahuasca - good thing about the people interviewed is they're all pretty respected academics and all of them are extremely positive about their experiences, acquired knowledge and peace of mind they have achieved since.

I went on a serious drug movie binge for a couple weeks, watching Permanent Midnight, Midnight Cowboy, Spun, Fear and Loathing, Trainspotting, Requiem for a Dream, Altered States, etc. and Requiem has got to be my favorite (with F&L coming in at a close second). Not necessarily for realism, but for the story and the characters. Sara Goldfarb, believe it or not, is my favorite character in the movie. The actress playing Sara did such an amazing job of portraying her downfall. Don't get me wrong, the other characters' stories were equally depressing, but the performance and delivery that she gave as Sara was absolutely stunning.

Those movies portray stoners in a negative stereotypical light. It's a kind of weed-bigotry.
How are we supposed to convince people that pot is less harmful than alcohol if we're okay with promoting the idea that we're all idiots because we smoke?

GUMMO is a sincerely disturbing movie on so many levels. Pretty sure I was sober (if not, just smokin' weed) when I saw it and I was horrified most of the time, lol. It's brutal, won't see it again, but don't regret seeing it actually. Never known anyone that has seen it or know what it is til now when you mentioned it, lol.
This one is a stretch for drug-themed, but the mother is a regular heroin user. Visitor Q, a Japanese live action film. Truly fucked up but in a lot of different ways ... One scene has the father of the family banging a female co-worker that he accidentally choked to death and brought back to his home. Rigor mortis causes his junk to get stuck inside the dead girl, and his wife eventually gives him a shot of heroin to free him. It's a lesser-known film by Takashi Miike (Ichi the Killer, Audition, and many more) who is known for his controversial films.

Gotta love those japs. I saw one movie that was really fucked up. This guys neighbour vanishes and at the same time he is banging some young bird at the university or something. Later on it turns out that she is a serial killer and goes after blokes who take advantage of young women.

As for Jacobs Ladder, the fact of having a "bad trip" is all you got from that movie???
They all killed each other in vietnam after being sprayed with LSD by over head planes. It really happened mate.
What you saw was him living his last few dying breaths and finally figuring out that he was dead thereafter.

so yea I am checking this out right now and I'll tell you I am tripping balls on nbomes. So the sound kicked in after about thirty seconds and so far its been very entertaining
edit: Yes I agree it's very fun to watch while tripping balls just the sounds and music from it is very trippy
