For Europeans this is the one and only:
https://bitcoin.de

Where does it say they payout with cash?
You can withdraw your funds with a few steps. Using SEPA, MtGox Code, Bitcoin. For SEPA we need 24 Hours to process your payout.
Only ones i know of that do cash in mail is bitcopia and bitcoins-to-cash.com, others will do money order/ cashiers checks, though they isnt anonymous if thats what you're after.

bitcoins-to-cash.com is down and bitcopia:
"Bitcopia is still working to resolve API difficulties but will be accepting orders via email. The maximum amount for cash in mail is $2000 per day. Please email bitcopia@gmail.com for a quote. Thanks!"
Not exactly sites i would trust my money to. ;)

The only one who knows where the cash went is the admin. I doubt he keeps records the way his TOS and legal info look.
You could even use a fake address and the only trace would be a random IP.

I can help you cash out. Please visit my vendor page for more information.

Not long after that message, they displayed another message about halting all trades because of further issues. As of right now, their site looks broken (doesn't load all the way). And Bitinstant is still down. Why can't these sites get their shit together? Certainly does not instill me with a lot of confidence.
I was going to try Bitcopia for the first time because BST is closed today, but screw that, I will wait for BST, they are reliable every single time.
