Maybe a box that smells really bad. Otherwise domestic there's near 0% chance it'll get caugh. Maybe a letter sorted ripped open and there's drugs everywhere.

I was taught there is no such thing as a stupid question. I firmly believe that to be true. The question you've asked might be more accurately described as ..perhaps misguided. Someone or thing has seemingly planted in your head...a false notion. The question can't be answered sincerely because it's based on the false premise that PO Boxes necessarily require special measures be taken to mitigate some unavoidable, inherent raising of suspicion not associated with other delivery options.
Taking delivery of illegal drugs via the USPS is inherently risky whether its to your home, office, PO box, anonymous drop..etc. There's really nothing suspicious about any of it...until you get caught, of course. :)
When I open a PO box I tell the clerk several times that I am not ordering drugs.
"By the way, sir, in case you were wondering, I am NOT planning to order drugs to my PO box. Drugs are for losers!"
This assures I am safe because now they know there won't be any drugs in my PO box.
