Are any of the sites providing any info about things hat have gone wrong? Stories keep occuring of markets getting hacked etc (though be as cynical as you choose) so is there any analysis of how things occurred, and what others need to check?

What went wrong?
OPSEC OPSEC OPSEC. literally can't say it enough. Deviation from the norm of a secure operation is what has done many people in, consistently.
Nobody we know of yet has been busted because their address wasn't encrypted.
All the software used has shown no issues with privacy, except JavaScript.
OPSEC.

Most of the time all we can do is guess whether is was a scam by the admins or an actual hack. Both are possible. All we can do to be safe is PGP everything, launder your coins and DO NOT KEEP FUNDS ON SITE.

The weakest point in these operations is the delivery, any other aspect is damn secure but the mail.
