From what I have gathered its physical address unless you set up your mail to be forwarded. Then you're fucked as then they have your new address.

Probably just the address. I have heard they keep records of the names attached to address. But I'm sure if your name is John Smith they could never tell who is who.
That's a good question and that would fucking suck if that red flag kept following you around!!!
